It starts with trust. Trust that staff can speak honestly. Trust that transparency won\u2019t be punished. Trust that when something goes wrong, our first response will be to ask \u201cwhat happened?\u201d\u2014not \u201cwho can we blame?\u201d<\/p>\n<p>The systems we build shape the behaviour we get. If we want aged care to be safer, more compassionate, and more responsive, we need to design for truth\u2014even when it&#8217;s uncomfortable.<\/p>\n<p>Because the cost of silence isn\u2019t just missed data.
